Social Work Senior / Medical Case Manager

The Bluegrass Care clinic is growing and seeking a new Social Worker/ Medical Case Manager to expand the case management team. This position serves individuals enrolled in the Ryan White Part B grant to ensure seamless continuum of care. The Ryan White grant service individuals living with HIV. This position allows one day of remote work per week after orientation period. MSW/CSW required. Position closes on April17th.
