We are seeking a dedicated Behavioral Health Professional to become a member of our diverse holistic care team! In this role, you will work as a member of the primary care team providing clinical consultation, assessment and brief intervention in the medical clinic setting.
Under routine clinical supervision, this position provides Behavioral Health consultation to children, adolescents, adults and families in order to improve psychosocial functioning. Clinical supervision is provided by a HealthFirst Bluegrass Licensed Clinical Social Worker, as applicable to this job description for the duration of employment in the job.
ESSENTIAL DUTIES and RESPONSIBILITIES
Provide comprehensive assessment and diagnosis of patients seeking behavioral health treatment.
Develop effective treatment plans, that include specific goals to be met and timeline for meeting goals
Assist clients in achieving established self-management goals
Evaluate crisis situations and apply appropriate interventions or referrals if needed
Actively participate in meetings that support an integrated Medical/Mental Health care model to provide comprehensive care for clients
Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
Assist in the management of chronic disease by working with patients in self-management of chronic diseases
Refer patients for other treatment when objective progress in patient’s condition cannot be documented
Work closely with teachers, school administrators, guidance counselors, and other Fayette County Public School System employees when warranted
Complete all Fayette County Public School requirements (background check, referral pathway, behavior on school property, etc.)
Provide trainings for staff and/or families on Behavioral Health issues as needed
Must comply with HIPAA, HITECH, and Part II regulations.
Participate in quality improvement documentation, including the use of symptom rating scales and peer review evaluations, as necessary
Meet HealthFIRST Bluegrass Inc. productivity and quality standards

SUPERVISORY RESPONSIBILITIES
This position does not have direct supervisory responsibilities.
EDUCATION and/or EXPERIENCE
Must maintain credentials as certified clinical social worker in the state of Kentucky
One year experience providing clinical behavior health services.
Must maintain compliance with licensure board continuing education requirements.
Integrated medical care model experience preferred

Computer skills – knowledge of word processing software
Excellent working knowledge of Behavioral medicine and evidence-based treatments for Mental Health and Behavioral Health conditions
Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of Mental Health and Behavioral Health conditions
Ability to communicate and work with an interdisciplinary team in a primary care center serving large numbers of low income and minority patients
General knowledge of the clinical effects and side effects of prescription and illicit drugs used by Mental Health and Behavioral Health patients
Ability to design and implement clinical pathways and protocols for treatment of selected chronic Mental Health and Behavioral Health conditions
SPECIAL KNOWLEDGE and SKILLS
This position requires effective oral and written communication skills, excellent interpersonal skills, and intermediate computer literacy.
LANGUAGE SKILLS
Incumbent must possess the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Incumbent must possess the ability to write reports, business correspondence, and procedure manuals; effectively present information and respond to questions from groups of managers, patients, other employees, and the general public.
MATHEMATICAL SKILLS
Incumbent must possess the ability to work with mathematical concepts such as probability and statistical inference, and the ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Incumbent must possess the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Incumbent must possess the ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
Must maintain credentials as a certified clinical social worker in the state of Kentucky. Must maintain mandatory continuing education requirements. Current CPR certification is required and must be maintained throughout the duration of this position.
PHYSICAL DEMANDS
Prolonged periods sitting, listening, and observing; visual acuity to inspect data and items, ability to hear and understand spoken communication.
WORK ENVIRONMENT
Will perform work in a clinical office setting; requires interaction with clients and public frequently; some clients may be emotionally unstable. May be exposed to blood or other bodily fluids. May require occasional weekend or overtime work.
