Social Work Senior/UKHC

The Infectious Diseases Division is looking to add a Social Worker Senior to its Medical Case Management Team. This position will ensure a seamless continuum of care for patients who receive medical case management through the Ryan White Part B program at the Bluegrass Care Clinic.

The Social Worker Senior will serve as a member of the multidisciplinary healthcare team in order to provide services to patients, perform and document psychosocial assessments, assist clients in accessing available medical and social services, empower clients toward self-sufficiency, coordinate the delivery of services, and assist with patient care in both the clinical and home visit setting.
