The Hope Center, Inc. is a well-respected and leading recovery organization in Lexington, Kentucky-the second largest city- that cares for the homeless and at-risk persons by providing life-sustaining and life-rebuilding services that are comprehensive and address underlying addiction causes.
PURPOSE
The Street Outreach Clinician will collaborate with the outreach team to identify those with mental health needs, provide emergency mental health services and assessments to outreach clients. The clinician will also provide psychoeducational groups for other Hope Center program clients.
JOB RESPONSIBILITIES
Responsibilities include, but are not limited to:
- Complete mental health assessments, including psychosocial assessments and diagnostic evaluation.
- Must be able to meet with clients in the field, in encampments or other locations where clients are living.
- Collaborate with the Street Outreach Program Coordinator regarding referrals and services.
- Keep client files and documentation up-to-date, including entry into the Homeless Management Information System and Dr. Chrono electronical health record.
- Make or arrange appropriate referrals to mental health or medical providers, as needed and collaborate with other case workers to ensure service needs are met.
- Participate in all scheduled trainings and staff meetings.
- Comply with all Hope Center education requirements and state licensure requirements, maintaining an active license.
- Work cooperatively with all referral sources, internally and externally.
COMPETENCIES
This position must demonstrate excellent written and oral communication skills, ethical practice, relationship management, critical evaluation, proficient computer skills, and the ability to work with persons in a way that shows sensitivity to cultural diversity, sexual orientation, and racial/ethnic differences.
EDUCATION/EXPERIENCE
Master’s degree in social services with at least one year of experience working with mental health, substance use or homeless populations.
CERTIFICATES/LICENSES/REGISTRATIONS
Independent licensure as a LPP, LPCC, LCSW or other approved license and ability to practice in Kentucky.
Current driver’s license, with acceptable driving record.
HOURS
This is a part-time position, regularly scheduled 20 hours/week. Employee, with supervisor approval, has the flexibility to schedule work hours during times that are mutually beneficial to themselves and the organization.

Pay: $35.00 per hour
