Field Agency Information

The mission of Field Education is to provide mutually supportive and positive learning environments that cultivate leadership, increase knowledge, refine skills in the core competencies, and strengthen social work values in student interns. Experiential preparation for professional practice is an integral part of social work education. Without our agency partners, the mission of Field Education could not be met.  We greatly appreciate your willingness to join us in preparing future professional social workers.

The agency approval process begins with an Agency Profile Form.  Completion of the web form below provides the necessary information to determine if the proposed practicum site meets the Council on Social Work Education (CSWE) accreditation requirements.  Once approved, the information will be used to set up your agency profile page in our intern placement database.

Field Agency Profile Form

An education affiliation agreement between the University of Kentucky and the agency is required by the University and College in order to place student interns in an agency. The purpose is to establish an agreement between the university and the agency regarding implementation of experiential education opportunities for students. All students are covered by professional liability insurance while enrolled in practicum and the affiliation agreement forms the foundation for this coverage. The Field Education office will initiate the affiliation agreement following submission of the agency profile form and approval by the Director of Field Education.
