The purpose of Field Education is to provide mutually supportive and positive learning environments that cultivate leadership, increase knowledge, refine skills in the core competencies, and strengthen social work values in student learners. Experiential preparation for professional practice is an integral part of social work education. Without our agency partners, the mission of Field Education could not be met.  We greatly appreciate your willingness to join us in preparing future professional social workers.

Agencies hosting students must complete an Agency Profile For, which provides the necessary information to determine if the proposed practicum site meets the Council on Social Work Education (CSWE) accreditation requirements.  Once approved, the information will be used to set up your agency profile page in Tevera, our online field management system.

A partnership agreement between the University of Kentucky and the agency is required by UK and the CoSW to place student interns in an agency. This establishes an agreement between the university and the agency regarding each partner’s responsibilities in the field placement process and also forms the foundation for student liability coverage, ensuring that all students are covered by professional liability insurance while enrolled in practicum. The Field Office will initiate the affiliation agreement following submission of the agency profile form and department approval.

If you have questions regarding practicum student requirements or are interested in the field education preparation process for our students, including field forms, please visit the Current Students page.

Sample Affiliation Agreements

These are sample documents and are not intended to initiate the agreement process.  You will receive information on the affiliation agreement process after your agency profile form is completed, submitted, and approved by the Director of Field Education.
