We are seeking a dedicated Behavioral Health Professional to join our integrated team! In this role, you will work as a member of the primary care team providing clinical consultation, assessment and brief intervention in the medical clinic setting.

Key Responsibilities:
Provide comprehensive assessment and diagnosis of Behavioral and Mental Health clients.
Develop effective treatment plans, that include specific goals to be met and timeline for meeting goals
Assist clients in achieving established self-management goals
Evaluate crisis situations and apply appropriate interventions or referrals if needed
Actively participate in meetings that support an integrated Medical/Mental Health care model to provide comprehensive care for clients
Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
Assist in the management of chronic disease by working with patients in self-management of chronic diseases
Refer patients for other treatment when objective progress in patient’s condition cannot be documented
Provide trainings for staff and/or families on Behavioral Health issues as needed
Must comply with HIPAA, HITECH, and Part II regulations.
Other duties as assigned.
Qualifications:
· ·Licensed Clinical Social Worker with current Kentucky license
· Must maintain compliance with licensure board continuing education requirements
· A minimum of 2 years of direct clinical experience
· Integrated medical care model experience preferred
